The Foundayo dose progression is:
0.8mg → 2.5mg → 5.5mg → 9mg → 14.5mg → 17.2mg
You will usually remain on each dose for at least 30 days before moving to the next strength.

Although Foundayo can be taken at any time, taking your tablet at approximately the same time each day can help you remember your dose.
This flexibility is one of the main differences between Foundayo and some other oral GLP-1 weight loss medicines, which may need to be taken on an empty stomach and require patients to wait before eating, drinking or taking other medicines.
Foundayo works by mimicking the effects of GLP-1, or glucagon-like peptide-1, a hormone naturally released by your body after eating.
GLP-1 plays an important role in controlling appetite, digestion and blood sugar levels.
By activating GLP-1 receptors, Foundayo can help:

Like other GLP-1 weight loss treatments, Foundayo can cause side effects, although not everyone will experience them. The most commonly reported side effects affect the digestive system.
Nausea, diarrhoea and vomiting are particularly common when treatment is first started. These side effects generally become less noticeable over time in most people.
Foundayo is used alongside a reduced-calorie diet and increased physical activity for weight loss and long-term weight management in eligible adults.
For weight management, Foundayo may be prescribed for adults who initially have:
a BMI of 30 kg/m² or greater (obesity), or
a BMI of at least 27 kg/m² but less than 30 kg/m² (overweight) and at least one weight-related health problem.
Weight-related health problems can include:
prediabetes
type 2 diabetes
high blood pressure
abnormal levels of fats in the blood
obstructive sleep apnoea
impaired heart function
a history of heart attack, stroke or blood-vessel problems

Before treatment is prescribed, a clinician will assess your suitability to make sure Foundayo is an appropriate option for you.
Foundayo is not suitable for everyone.
You should not take Foundayo if you have had a serious allergic reaction to orforglipron or any of the other ingredients contained in the medicine.
You should speak to your doctor, nurse or pharmacist before taking Foundayo if you:
have or have previously had pancreatitis
have severe problems with food digestion or food remaining in your stomach for longer than normal, including severe gastroparesis
have kidney problems
have gallbladder problems
have a problem with your eyes or vision, such as diabetic retinopathy or macular oedema
have or have had severe heart failure
are taking insulin or a sulfonylurea for diabetes
are taking medicines for high blood pressure
are due to have surgery where you will be under anaesthesia

Foundayo should not be used during pregnancy because its effects on an unborn baby are not known.
If you are pregnant, think you may be pregnant or are planning to have a baby, speak to your doctor before using Foundayo.
If you could become pregnant, it is recommended that you use contraception while taking Foundayo.
Orforglipron may reduce the effectiveness of oral hormonal contraceptives. If you use an oral contraceptive, you should consider using a barrier method of contraception, such as condoms, or switching to a non-oral contraceptive method for 30 days after starting Foundayo and for 30 days after each dose increase.
If you wish to become pregnant, discuss your treatment with your doctor. Foundayo should be stopped at least 3 weeks before trying to become pregnant.
Foundayo should not be used during breastfeeding.
It is not known whether orforglipron passes into breast milk, and a risk to newborns and infants cannot be ruled out.
Speak to your doctor if you are breastfeeding or planning to breastfeed before starting treatment.
If you forget to take Foundayo, take the missed dose as soon as you remember on the same day.
If you do not remember until the following day, skip the missed dose and take your next dose on your usual scheduled day.
Do not take two doses on the same day to make up for a missed dose.
If you miss a dose for more than 7 days, take your next dose on your regularly scheduled day.
If you are unsure what to do after missing a dose, speak to your doctor, pharmacist or clinical team.

Foundayo (orforglipron) has been studied in large clinical trials involving adults living with obesity or overweight.
One of the main studies supporting the use of orforglipron for weight management was ATTAIN-1, a 72-week Phase 3 clinical trial involving 3,127 adults with obesity, or overweight with at least one weight-related health problem, who did not have type 2 diabetes. Participants received orforglipron or placebo alongside a reduced-calorie diet and increased physical activity.
The study found that average weight loss was greater with higher doses of orforglipron:

These results are based on the study's analysis of average percentage change in body weight at 72 weeks. Individual results varied between participants.
The highest dose also produced clinically meaningful improvements in other measures associated with cardiometabolic health, including waist circumference, non-HDL cholesterol, triglycerides and systolic blood pressure.
A separate analysis using an efficacy estimand found an average weight loss of 12.4% after 72 weeks among participants receiving the highest dose. This figure should not be interpreted as a guarantee of the amount of weight an individual will lose.
Weight loss results vary between individuals. Your response to treatment may be affected by factors including your starting weight, dose, diet, physical activity and how your body responds to treatment.

Foundayo and the Wegovy Pill are both once-daily oral GLP-1 receptor agonist treatments used for weight management, but they contain different medicines and have different instructions for how they are taken.
Foundayo contains orforglipron, while the Wegovy Pill contains semaglutide.
One of the biggest differences is the flexibility around taking your daily dose.
Foundayo can be taken at any time of day, with or without food.
Wegovy tablets must be taken on an empty stomach after a fasting period of at least 8 hours. The tablet should be swallowed whole with a small amount of water, and you should wait at least 30 minutes before eating, drinking or taking other oral medicines.
These different dosing instructions are related to the way the two medicines are absorbed in the body.
Both orforglipron and oral semaglutide have been studied in clinical trials and have produced significant weight loss when used alongside dietary and physical activity changes.

In the OASIS 4 trial, adults with overweight or obesity without type 2 diabetes who received oral semaglutide 25 mg once daily alongside lifestyle intervention had an estimated average weight reduction of 13.6% after 64 weeks, compared with 2.2% with placebo.
However, these results come from separate clinical trials with different study populations, treatment durations, study designs and medicines. They should therefore not be interpreted as a direct head-to-head comparison of Foundayo and Wegovy tablets.
The most appropriate treatment will depend on factors including your medical history, other medicines, treatment preferences, response to treatment and the dosing requirements of each medicine.
Foundayo can interact with certain medicines, so it is important to tell your clinician about all medicines you are currently taking before starting treatment.
This includes prescription medicines, over-the-counter medicines and supplements.
Foundayo slows the rate at which the stomach empties. This means it may affect how quickly some medicines taken by mouth are absorbed.
Examples of medicines where particular care may be required include:
insulin
sulfonylureas or other diabetes medicines
simvastatin
oral contraceptive tablets
certain antibiotics
certain antifungal medicines
medicines that affect how drugs are processed by the liver
Taking Foundayo alongside insulin or some diabetes medicines may increase the risk of low blood sugar. Your diabetes medication may therefore need to be reviewed or adjusted.
Some medicines can also increase or decrease the amount of orforglipron in your bloodstream, which may affect how well Foundayo works or increase the likelihood of side effects.
Always provide a complete and accurate list of your current medicines during your consultation so your clinician can check for possible interactions.
